What insurance plans do you accept?

Accepted Answer

We accept all insurance plans except the following: Cigna/HealthSpring; PrimeWell; Shared Health; UHC HMO; UHC DSNP/Dual Complete; and AARP Medicare Advantage from UHC.

UnitedHealthcare Medicare Advantage PPO is accepted, but we are out of network, so patients typically have a higher copay and deductible.

Do I need a referral to see Dr. Black?

Question 4

What should I bring to my first appointment?

Accepted Answer

Bring your insurance information, photo identification, medication list, and any relevant medical records. Previous X-rays are not necessary, as new X-rays will generally be obtained at your first visit.

If an MRI, CT scan, or other advanced imaging study was performed outside the Memorial system, please bring the actual study on a disc, in hand. DO NOT RELY on the study being sent to our office on your behalf. The imaging report is also helpful. If you have had previous surgery, bringing the operative report is very helpful as well.

What should I expect at my first visit?

At your first visit, we will obtain a baseline set of X-rays, review your history, and perform an examination. You will then have a discussion with Dr. Black regarding your specific situation. Dr. Black will develop a treatment plan based on your unique circumstances and goals.

Question 6

Should I get an MRI before my appointment?

Accepted Answer

For knee patients, an MRI before your first appointment is generally not recommended. For shoulder patients with a significant problem, having an MRI ahead of time can often be very helpful and allow more to be accomplished at the first visit.

If your primary care physician can obtain an MRI before your appointment, that can be beneficial, but it should not delay your visit. If the MRI was performed outside the Memorial system, bring the study on a disc, in hand, along with the report if available.
